What if one natural product could help your patients sleep better, recover faster and reduce their need for pain meds? The research says CBD might just do all three. Welcome back to the Factor Podcast. I'm your host Jessica Riddle and today we're diving into part two of our conversation with Dr. Clint Sellars. Jump into kind of in office stuff and key patient needs. These are my again, these are Ananda products because that's who I work with, but these are kind of the top five that I use in office and why I use them in office. So if you're looking for just a straight CBD product, they do have a complete core softgel in my world, working more in the the musculoskeletal realm. The influence line is really kind of where it's at for me. Definitely my favorite. That line is designed specifically for musculoskeletals so it'll reduce inflammation. It supports muscle joint overall body comfort. I've gotten some really good results with patients using that outside of the office, but also inside the Office they've got a roller that I think is amazing.
I have a lot of patients that come in and they don't want biofreeze or the more of the creamy smelly topicals 1. They especially office workers that need to go back to work. They don't want to leave the office smelling like a gym, they say. So this product has no smell, goes on very clean, dries. There's no filmy residue, anything like that from a taping world. It's much better for me to use versus a cream. The tape sticks so much better. I don't have to worry about cleaning them off, that kind of thing.
But you can get some really targeted relief from that roll on versus just putting a cream on that's going to warm their skin. Side note, for CBD topicals, the CBD will not reach the bloodstream. So if anybody comes in and they're worried about that from a drug testing or something like that, which we'll talk about in a little bit, the rollers of the topicals will not reach the bloodstream. The somnivive product up here on the top right, that is that sleep relaxation and a patient where they want that natural sleep help or that natural relaxation help. I've had some very good results with that product. And then the cogni active on the bottom right here is quickly becoming number two or or tied for number one and my favorite. But that product is designed for boosting brain function and vitality. So your energy patients focus patients especially since COVID a lot of people talking about brain fog.
That product I have seen some amazing results with. And here's another study that ties into our clinical use here it was the Journal of Pain Research. They studied seven pain clinics. And moral of the story here is that they found among like adding CBD they had a 64% of those studied said that they've had relief of their back pain. Another 40% nerve pain improvement, 40% neck pain improvement, 30% migraine improvement. And the list goes on. Right. So this is without other therapies.
So if we're doing the normal things we're doing in office, we're adjusting, correcting movement patterns, taping, scraping, all these things. These patients or these people in this study, they're reporting what almost two thirds of them had relief of their back pain just by using that CBD product. So this is huge potential for us to offer a supplement or an option to keep people out of pain outside of the office. But to also really add that value added part to our practices of. Let me Help fix the underlying condition. But this can also help get you out of pain, get you sleeping better. Sleep is a big part of recovery, right? So if a patient's not sleeping, we can adjust them all day, but they're not having that restorative time to really let their bodies heal. We need to really be thinking about what else can we add or how can we supplement or support what we're doing in office.
These numbers are telling us that these products really can be that for our patients. This was a really cool, really cool results here. The main medical center did a study of 1500 opioid users and these were chronic non cancer pain patients. They saw a reduction of almost 80% in opioid usage after adding a CBD product. 72% reduction in anxiety meds, 37% reduction in depression meds, 65 in sleep meds, 66 in migraine meds. And they actually saw a 42% decrease in alcohol consumption. Again, these numbers are looking pretty amazing. This is an Ananda study telling us that same thing.
But they did an evaluation of CBD extract from opioid use and quality of life in chronic pain patients. They found that 94% of patients reported better sleep and less pain. 53% stopped or decreased opioid use after adding Amanda products. Pretty cool stuff there. This is just kind of an overview of some other sleep studies. There's some really good results here that you can see, but a couple of them. CBD significantly increased total sleep time, including REM sleep. CBD suggests that it has potential in treatment of insomnia, especially in ptsd.
CBD decreased anxiety scores, increased sleep scores. Lots of great results from quite a few different studies. There again, the importance of sleep in healing and restoring the body. And then as far as cognition goes, Amanda did a study on Cogni Active. They did over four weeks. They studied omission and commission errors, which basically boils down to people not doing something that was they were supposed to do or doing something wrong. When in the study they saw great improvements in both of those. Big increase in processing speed and then they saw a 143% increase in BD&F production.
So a lot of good results there. There's a lot going on on this slide, but the importance of this is focus. Focus has become a huge growing market in our society. I think a lot of that has probably stemmed from COVID but you'll get a lot of these brain fog patients or lack of energy patients. The global brain health and energy and cognition market is growing big time. This was A study they did multiple different demographics. But in this study, the big picture is energy or lack of energy was top five among every group that was studied. Currently we're only really solving that problem with Adderall and caffeine.
And patients aren't wanting the caffeine because it's affecting their sleep. There is a need and patients are asking for, and they are looking for natural energy boosters, natural focus improvement, that concentration, all the above. I have a lot of patients ask me about it, but the numbers are showing in general, that is a huge growing market right now. Dosing and home care. That's like I said before, that's the number one question or concern I get from patients is how do I use it, what's my dosing, can I use it, that kind of thing. So we'll jump into that. Dosing can be hard, it's very patient driven. It can be different based on weight, severity of the issue, number of receptors they have in the body, which can boil down to recreational usage of things or cbd.
In the past, how long has it been since they've taken in any kind of an endocannabinoid? The one thing to really remember we've kind of already hit on was that if you don't use it, you lose it. So what we really want to do with dosing is start small and then ramp it up. So going back to quality brands, how do we know who we can work with? And the transparency idea. A good reputable brand should have the instructions and the how tos included with every product. I've seen quite a few out there that literally don't say anything. You pull the dropper out, there's no lines on it, there's no instructions, there's no anything. This is the one of those infleas products here on the left of the slide and it literally has the dosing instructions right there on it. This is also where patient education comes into play for us because it can be so patient specific.
Severity of the issue can play a role. What we're treating can play a role. General guidelines, what Ananda recommends and kind of the science tells us is that let's start with like a half dose for the first couple days and then we're going to ramp it up. We want to get those receptors in that endocannabinoid system ramping back up and ready for the treatment. Right. So first couple days, half dosage, then we start to ramp up. The thing you'll find with CBD is that you'll get as you ramp these Doses up, you'll get increased benefit, increase benefit until you don't. So you'll have this big drop off once you go too far with the dosing.
So if you're at 35 milligrams, let's say everything's great, you're feeling awesome, you jump it to 40 and all of a sudden all those benefits go away. We know that you've passed it, so we need to go back to the 35 and that's kind of your sweet spot. That's where we need to hold your dosing at. These on the bottom right here, these are kind of general guidelines for different conditions. And again, severity is going to play a role in that. And then as far as timing goes, it also can be very symptom dependent on that front. A pain patient may want a higher dosing or a max dosage first thing in the morning when they wake up. And we know that the half Life is roughly 8 to 12 hours.
So they may also need more 8 to 12 hours later, just like they would with an over the counter. But they might want to take their max dosage early in the morning sleep. Patient obviously probably wants their max dosage before bed. Later at night, somebody that maybe they've got a few different conditions or maybe they're having trouble sleeping, but they also want it for stress. They may want to divide that dosage up throughout the day to get more of kind of a consistent intake. So just remember there, the important thing too is that starting dose is not designed to fix the problem. It's designed to ramp up the system and get it ready. So a lot of the patients, you'll hear, I tried CBD and it didn't work.
One, they might not have been taking enough and two, they might not have given it enough time for the body to be ready for that product. Pregnant patients, few of the other kind of sensitive populations we should talk about quick. Animal data right now is showing no negative effect at doses below 150 milligrams per kilogram of body weight. So to put that into perspective, for a 150 pound human, that's 10,000 milligrams per day. So animal data is looking like it's pretty safe, right? Really safe. But ethically not a lot of studies have been done there. So there is limited data on human trials. So we can't just, like a lot of other things, we can't 100% guarantee safety or guarantee anything.
But informed consent plays a big role here. The animal data shows this. Weigh the risk and reward with your patient and let them make a decision from there. Breastfeeding CBD can get into breast milk. The relative dosage seems to be around two and a half percent and the general guidelines for safety or anything less than 10% is generally safe. Again, not a lot of studies done from an ethical perspective here. So use that just the same as a pregnant patient. The data shows this.
Weigh your risks and rewards, let them make a true informed decision. And drug tests are another big thing to talk about. A lot of patients ask about this. The urine drug tests can't generally determine THC versus CBD effectively. So you will get a lot of false positives with really any CBD product that you take. What you can do from there, if you do get a positive on a urine test, you can request a serum blood test and that will generally clear up any false positives that may come out. There's always a chance with a full spectrum that you could get a true positive test. But again, the benefit of partnering with a good company like Ananda specifically, I keep talking about them because that's who I use.
But they've got doctors on staff that they've worked with for a long time that have written letters that you can provide to employers that talk about the false positives, how they can occur, what the treatments are doing, that sort of thing. So the options are out there specifically for like a law enforcement or a pilot that has to get tested regularly. There are ways to communicate with the employers and the blood test is also a really good option to clear up any false positives. And then children, the who says that naturally occurring CBD is safe for children. What we generally recommend or definitely recommend is broad spectrum for any kids less than 12 or 14 years old. Just so we're not providing them THC in those growing years. You can give them a full spectrum product if needed. But again that's a risk reward, patient specific thing.
Generally though, who says it's safe? Just be careful with the thc. And then this is just a little overview of some of the studies that Ananda's currently doing. Like I said it before, they're really trying to push the, the envelope and advance the profession as far as CBD goes. There's a lot of cool research they're involved with that. I've seen some really cool stuff on endometrial cancers, but they're, they're constantly trying to put out new and better products and, and really do a lot of the research the, the industry right now. The reason that there's not a lot of research done is because there's not funding for it. So the people that are going to do the research, the companies that are manufacturing it have to front their own money to do the research. And not a lot of them are willing to do that.
So it's pretty cool to see a company that's out there that's really pushing the envelope as much as they are. On that note, a few other things I was thinking of before. As far as the market goes. There's been a lot of Talk lately on Delta 8 vs Delta 9 THC and there some people are, are trying to say, oh, Delta 8's better because it's not, it's not gonna do X, Y or Z or vice versa. But generally Delta 8 and Delta 9 are, are very similar. Delta 8 is like the Bud Light version or Delta 9 would be like the Bud Heavy version. It's just weaker. It doesn't last as long, but you still will get the intoxication effects, that sort of thing.
And then federal and state laws are ever changing. So talk to, talk to your boards, talk to your state, figuring all that out. Ananda can help with that. They have people that are constantly working on that, trying to advance the regulations. But I have a rep there I can specifically talk to about rules, regulations, what's changing in the market. So if you need help, they are source, but also check with your boards, that sort of thing.

So to start with going back to.
The questions about titration and dosing, you could you talk us through or kind of walk us through a typical patient visit that you have where you start to introduce CBD as a potential treatment or a potential adjunct to an existing treatment plan that you have with a patient? What does that conversation look like just so that you can give them a comfort level and let them know what to expect?
Oh man. I mean, a lot of it is just the education on what we just talked about. So I'll typically talk to them about hemp versus CBD because a lot of them do have questions about is this going to make me high? Is it going to pop on a drug test, Is it going to affect my driving? Lots of questions similar to is this intoxicating? So educated them on the difference. This isn't going to intoxicate you. These are the different options of products. Like you can get products that literally have no THC in them but will still give you plenty of therapeutic benefit. Or there is a product that has 1/3, up to 1/3 of a percent of THC in it. So that would be microdosing a beer kind of, but that's not very specific.
But they kind of understand that at that point. Like, oh, that's a very, very, very small amount. Then it's kind of depends on what, what they're looking for. You start to talk about your whole body runs on these These receptors in this endocannabinoid system. So from a systemic perspective, like you might have knee pain or joint pain, but you're super inflamed or you're not sleeping. What. It's a lot of questions really. Like, what else do we need to focus on? Like, and, and that can be in, in an intake form.
I've actually been playing around with that recently, like, what are your goals? What else are you, you not dealing with? I know it gets hard for us to really expand too far out of our traditional scope of practice or our traditional wheelhouse. But even putting some questions in an intake form of are you looking for sleep or nutrition or any of these other things is a good way to just kind of open up the talks of these products can help you with sleep, anxiety, all of these things together. So, you know, you'll get some joint pain relief, but it'll also help you sleep, it'll also help with your anxiety. So I don't tend to go straight into a CBD talk or specific CBD talk. I try to kind of blend it in with normal patient visits. Like, have you tried this before? Here's why this product's different, here's how it can help you. Here's why I chose this company to work with a lot of, a lot of patients really respect and appreciate knowing that, that we're kind of the, the police, so to speak, on what we're offering. If I'm putting my name to it, it's, it's got to be a good product.
That help? Does that answer the question? I think I went on a tangent there.
No, absolutely. And I think kind of a follow up to that someone was asking about, when you're making those recommendations, do you typically have a patient start at the minimum dosage on the bottle and then kind of gradually work up. And I know everyone's different in terms of the dosage that's right for them. I've even heard that sometimes if you have too much, it can be ineffective for you, as you mentioned earlier, if those receptors aren't turned on and ready. So how do you recommend stepping a patient into their correct dosage and helping them titrate up or down to where they need to be.
I'll start them. Most people really with the recommended, like Ananda that I had on the slide there with those recommended doses per kind of symptom. But it, it kind of really depends on their severity. So somebody that's in a lot of pain, I won't start them at that lower dose. I'll start them at a higher dose because one, it'll kick in the receptors, but it also provides them more kind of upfront because if they are at that more severe pain level, starting them at a super low dose is just going to frustrate them and kind of throw their money away. So they're going to end up being one of those patients that's probably on the higher end of those dosing recommendations. So I'll start them higher to get to that point faster. This kind of an average patient, I'll follow those guidelines pretty close and then I'll.
I'll educate them. On day one of this is, this is what you need to be doing. It's going to take a few days. Like this first dosage is you're really not going to get anything out of it, and that's okay. Don't get frustrated by it. Then you'll start to titrate up, you'll see the benefits. And I'll generally have them step up by about 5mg every couple days and, and really make sure to educate them on. You'll get those benefits, benefits, and then you'll hit a wall and they'll all go away.
So then you back up five and everything should come back because you've already built up the receptors. Everything should be fine. From that standpoint, the next day, everything should be pretty well back to normal. Does that make sense? Is that helpful?
Yeah, I think that that absolutely makes sense. And I think it's important, as you mentioned, that you can inform them that there may come a point where they hit a wall. And if so, that doesn't mean that the treatment's ineffective. That just means they have to kind of back. Back down to where they were when they were receiving results.
Patients like to kind of own their sweet spot too. Like, it kind of becomes a game for them to find their perfect dosage. Like, ooh, I'm going to have my own dosage. Like, that's pretty fun for them most of the time. So they'll come in and they'll be like, I hit that wall you were talking about. And then I backed up and like, everything's great. Like, all right, that's cool. Work the way it's supposed to.
Yeah, absolutely. And I think more and more, as just the public becomes aware of their health, there's the biohacking movement. There's so much information now about natural and alternative means to help people beyond the pharmaceutical, beyond the surgical, especially when it comes to chronic pain, musculoskeletal issues. And dysfunction. It's important, I think, that people feel like they have that ownership over what they're putting into their body and what they're taking and of course, the dosage as well. Has there ever been a time where a patient received a result that was surprising to you? Maybe something that they had as a secondary issue was resolved or saw improvement that you weren't expecting and you were kind of like, oh, well that's, that's kind of cool that this was something that was effective for that thing.
I think more on the kind of brain function side before that that Cogni Active product came out, that's been fairly recent. I did have quite a few patients that came in and you know, reported, I feel like I can concentrate or I have more energy, that kind of thing, which, which I definitely was not expecting. Like we're talking about your knee pain or shoulder pain and how to get your pain to go away. And they come in and they're like, I feel less stressed or less anxious or the, the cognitive function side of it was, was pretty cool, let's say. That's probably, probably the biggest ones I've seen. I can't think of any crazy outliers off the top of my head.
Well, and it's interesting too because I think many of us, when you think about cbd, you think about hemp based products. I think things that come to mind because they're more talked about in terms of the benefits from a medicinal standpoint have always been the relaxation, the anxiety, the stress relief, the helping with inflammation. But it's interesting and it's kind of almost counterintuitive to think about these products that can help you with relaxation and help alleviate stress, can also give you energy and help you focus more. Because I think for so many of us, we kind of simplify those as two different ends of a spectrum. Right. So you have the things that hype you up and help you focus and have energy and the things that help you relax, unwind and de stress. So I think it's really interesting and unique that there are products out there that can do both. Especially because there are so many people that are dealing with some neurocognitive dysfunction issues, challenges.
Lord knows, all of us deal with stress and anxiety from at one point or another. But it's nice to know that there's a product that can kind of speak to both of those. So the patients that were seeing those benefits before that Cogni Active specific product came out, were they on one of the broad spectrum or full spectrum products? Were they. On one of the Inflamma products, do you remember which one of the products they were taking? And still seeing those.
Those were infleas products for sure. And that was pretty early on learning all about them too. So getting that understanding of the entire system and we get, we can get tunnel vision in office, right? And you're thinking musculoskeletal in fleas, like, how am I going to attack this thing that they came in asking for? And then you plan on, let's take care of this, and then we can talk about these other things. So it. There were a few times early in that let's, let's clear up your pain where they're coming in and they're telling me those things like, oh, that's pretty cool. And then a lot of patients will, they'll use their, their snooze button as a, as an indicator. Like, I used to snooze for an hour before I get out of bed, and I didn't want to get out of bed. And they're like, since I started taking that product, I wake up and I pop out of bed and it's like, all right, that's cool.
Like, why? Well, you have more energy, you're more focused, you're more motivated, that kind of thing. So that was pretty cool then really starting to expand and get into the different products and why to give different products was fun, but early on, that was pretty cool to see and have that front and center learning. Rather than somebody telling you this is a good sleep product or this is a good energy product, you can see it working in your patients without even intending it. You know what I mean? Pretty cool stuff.
Yeah. Positive byproducts and side effects are always a good thing, for sure of any treatment. So walk us through your. And I don't want to say typical patient visit because no two people are the same and everyone's an individual. But someone comes in, they're dealing with chronic pain or maybe some joint pain, musculoskeletal issues. How do you choose? Or do you combine the use of the tincture, the gummies, do you use a topical cream or have them use that at home? The roll on. How do you combine these different, I guess, mediums or these different. I don't even know what you would call it.
The, the different products in their different forms. Right. Because some are, you know, the liquid drops you put under your tongue. Some are gummies. I don't know if that's more of a preference of the patient in terms of what they like to take or what they will take. And then how do you choose? Combining that with some of the topical products that might be more very highly directed to a specific body area for a specific area or a point on their body versus the internal. And, and do you combine the two?
I do from a topical perspective. That one's, that one's easy. Like we. I don't know if, if you know much about Raleigh, but there's a lot of runners in Raleigh. So a lot of. A lot of muscle pain walks through the door and I'll just ask them are you using biofreeze? These kind of things. And if they say yes, like I'll immediately start talking to them about the roller just because a. A lot of people love biofreeze and, and we don't want to talk bad about anybody, but it's not the cleanest product on the market just, just by.
By what it is. Right. Like Icy Hot, Bengay, a lot of these other options that patients. I'm just asking them are you using these things? Are they helping? Most of the time they say they're using them but they don't like them. And I take that as a, as a. It's my job to try to get you away from these things that may or may not have chemicals in them and provide you more natural product. So that alone I'll. I'll immediately start talking about the roll on.
I have some in the office that I'll use on patients if they want it. But I start talking to them pretty early on about being more holistic. This product has much more therapeutic benefit. It lasts longer to me. And again the smell factor. A lot of them want to get away from the smell factor.
That's the thing. I think with any of us that have used those type of products, nobody wants to walk around smelling like Ben Gay, that's for sure.
And same same with the salve. Ananda has a great salve product that I'll use for instrument assisted stuff rather than potentially a cream that's got chemicals in it. Again I want to know what goes into that product. And a lot of the stuff on the market, I don't feel like they're nearly as transparent. So with the goal of moving them to a more holistic product line, I'll kind of immediately go there. But then all. All combined with in fleas specifically for if it's just a pain patient like I'll talk to them about here's a roller that really provides that therapeutic benefit instead of a biofreeze. But there's also a systemic product that you can take that will do all of these other things and it will increase and speed up.
Like One slide said, 65% of back pain patients just by taking that product felt better. So we are in office, we are trying to figure out and we are trying to fix the underlying condition, like what got you here. But this is going to help us and add a lot of value to it. But a lot of that too is talking to the patient and figuring out what they truly need. Like maybe you walked in with pain, but your biggest issue right now is that you're not sleeping. Like you're a crossfitter. You're going 100 mile an hour all day long. You work all day or work all night.
You get two hours of sleep because you have, you can't shut your brain off, that kind of thing. Like to me that that patient needs more of the sleep product than the musculoskeletal product because they're not recovering, they're not sleeping. So asking a lot of questions and trying to really dial in what they need, then I'll start to think about what product could really fit in there. But for the most part I would say my main ones would be the in fleas, the sleep and then really starting to get into the brain function side of things.
Absolutely. That sounds very, very cool. And it's always nice to kind of have an idea of real world application and how you're using this with patients because I know sometimes it can be overwhelming to see a number of different products and just kind of knowing where to start or how you combine those. We have just a few minutes left guys, so please be sure to write in with any final questions that you have. I do have a little more here. Someone was asking about kind of the the future or what you foresee when it comes to CBD and hemp and its use within medical and obviously within chiropractic and more natural healthcare realm realms. You have a bit of an insider perspective, having worked with these products for quite a while and having the relationship with Ananda. What do you think's in the future? Like what's up ahead that we could be excited to see more of? I know you mentioned on an earlier slide a few different studies that are underway now, but where do you see this going in the future and will more and more clinics start having this type of product as a treatment available for their patients?
I sure hope so. I think the only thing that could get in the way CBD based treatments would be the regulations and a Lot of that I think is just because it's, it's a really old or it's been used for like we said 8,000 years but then it wasn't for a long time. So it's kind of back in its infancy of really the potentials of it. But the, the potentials of it are pretty astounding with what has been researched so far. One of the coolest ones I've seen. Ananda did a study which was really one of the things that opened up my eyes to them was on endometrial cancer. And I don't know that they've published this study yet but they were looking at endometrial cancer and high level CBD as a treatment and got amazing, amazing results. I don't want to specifically speak to it because I don't know if they have published it.
But there, I don't know that you really even can answer that question because there's just so many huge potentials for what's out there. The endocannabinoid system is so intimately part of our biological functions and those receptors are, are spread out in all these different areas. Like it's kind of the sky's the limit as long as nothing gets in the way. Farm. I would love to see it start to really replace a lot of pharmaceuticals. That's not going to happen. A hundred percent know that. But why not have good holistic options versus synthetic pharmaceuticals? Will we ever get there to the level? I'd love it.
I have no idea. I would love if it did. But I can see, I can see the huge potential of Cairos pts, athletic trainers. Like even the simple things like soft tissue emollient like why not use a therapeutic emollient versus coconut oil or something or china gel or whatever, whatever is out there on the market. But instead of over the counter advil, why not CBD? It's completely 100% natural versus taking some of those other things. So I would love, and I do foresee that becoming a thing. Like you've got this CBD product that you could take for your pain relief instead of Tylenol. But look at these other benefits you get with it too.
Now you're sleeping better, now you have less stress, less anxiety. The Ananda team I work with could tell you a lot more about what's in the pipeline that I'm not aware of. But from what I've seen, I've seen some really cool increases in availability, usage, that kind of thing. That has also really stemmed the bad actors too. So that's where it really becomes important to know, know the company, know what they're doing and how they're doing it. So you know what you're, you're recommending just like anything that it's become much more popular. So it's also become much more wide variety of actors out there, so to speak.
Absolutely. I had someone a few years ago when CBD really first started to come on the market as something that could be used for medicinal and therapeutic purposes. At least the public knew about. Someone said it's like the wild, wild west right now in terms of regulatory restrictions from state to state and even on the federal level. One thing that's been exciting for me to see is for these specialized products like the ones that you were speaking about with Ananda, where they have a product that's specialized for sleep, where they have a product that specialized for cognitive health. Because from a patient perspective, it's really overwhelming to try to decide when you're looking at a bottle and it says 900 milligrams or 500 milligrams or even from brand to brand, they'll speak about the dosage differently. So one company will say it's this many milligrams and then another one will list it by a specific other measurement. And it's, it's just a little bit overwhelming, especially when you don't know the difference between full spectrum and broad spectrum and the isolates and things like that.
So we appreciate you giving us kind of a master class in that today and teaching us more about the endocannabinoid system and how you use this in patient care. I think that we've all walked away today with some great new information and have a little bit better idea hopefully of how we can all utilize this in practice with patients.
